SOCRATES

SOCRATES is an assessment that allows you to assess pain. This is to be used when requiring a more in-depth assessment of the pain the patient is in, instead of the 0-10 scale.

S - Site - Where is the pain?
O - Onset - When did the pain start? Did it happen gradually or suddenly?
C - Character - What is the pain like? Stabbing, aching, dull?
R - Radiation - Does the pain radiate to any other parts of the body?
A - Associations - Ask if there are other symptoms which are associated with the primary symptom.
T - Timing - Clarify how the symptom has changed over time.
E - Exacerbating and Relieving Factors - Ask if anything makes the symptom worse or better
S - Severity - Assess the severity of the symptom by asking the patient to grade it on a scale of 0-10
