Body Access

In order to give many medications you will require some sort of body access whether that be a:

  • Intravenous Line
  • Intraosseous Line
  • EVJA

IV:

  • Select needle gauge
    • 16 gauge - 180 ml/min - Rapid fluid, trauma
    • 18 gauge - 90 ml/min - Rapid fluid, trauma
    • 20 gauge - 60 ml/min - Most infusions, rapid fluid, trauma
    • 22 gauge - 36 ml/min - Most infusions, rapid fluid, paediatrics/elderly
    • 24 gauge - 20 ml/min - Most paediatric and elderly infusions
  • Find viable vein
  • Apply TQ 2-3 inches above the site
  • Clean site
  • Insert until blood flash
  • Upon observation, remove the needle, advance the catheter
  • Secure
  • Flush w/ 10 ml NS

IO:

  • Common locations of insertion:
    • Tibial plateau (2 cm below knee)
    • Humerus head
  • Other locations:
    • Distal femur
    • Clavicle
    • Radius 
  • Needle sizing
    • Red cap - paediatric 
    • Blue cap - normal adult
    • Yellow cap - longer adult (difficult locations, i.e. distal femur)
  • Attempt IV
  • Palpate area of insertion, rule out deformities 
  • Sterilise w/ chlorhexidine 
  • Attach selected size to drill
  • Re-palpate location, placing finger over site
  • Place needle where finger was, and plunge into the bone
  • Activate drill, drilling down until cap fills with flesh
  • Remove drill
  • Attach syringe, draw back bone marrow
  • Flush w/ NS
  • Secure cap

EJVA: 

  • Attempt IV, IO
  • Tilt head to side and away from intended site 
  • Identify jugular vein
  • Sterilise between neck and collarbone w/ chlorhexidine 
  • Drape incision area
  • Locally anesthetise using lidocaine, leave 1 cc in syringe 
  • Take 12 gauge needle w/ 1 cc lidocaine
  • Insert needle at 18 degrees towards heart
  • Advance until it hits jugular vein, shown through aspiration of lidocaine 
  • Detach syringe, thread guide wire through needle
  • Remove needle, hold guide wire steady 
  • Make a small incision moving away from the wire with an 11 blade 
  • Take the dilator and thread it over the guidewire, pushing it down as far as it can go, then remove it 
  • Threat the central line over the guidewire, remove the guidewire
  • Flush each port w/ 5 ml saline, clamp off, secure line
